Diego Simeone's side unbeaten start to the La Liga season -- 18 league games, including last season -- was put in doubt when La Real took the lead.
Jan Oblak gave away a penalty in the first half, brining down Oyarzabal, and Willian Jose dispatched from 12 yards.
But the home side were a different side in the second half, with Koke and Saul Niguez particularly inspirational as they came back to win 2-1.
Angel Correa had a contender for miss of the season in the 49th minute and Antoine Griezmann had a brilliant goal ruled out for offside in the 52nd.
Atletico's reward eventually came in the 63rd minute when Saul created the equaliser for Filipe Luis.
And as everyone was beginning to settle for the draw, Saul, once again, turned creator, providing the winner for Antoine Griezmann with just two minutes to go.
